How often does the #5 main, cause any problem or look like it didn't get as much oil as the others?

Not usually any sort of issue.
We once had a 390 where the drilling feeding #5 from the cam was not made - simply "not there". Engine showed signs of a couple rebuild attempts and very low service hours. I wagered that it found its way into a warranty core pile somewhere when it was determined that it could not be fixed.

I once broke down a 390 that had notable trauma to only the rear main bearing and journal. It also had the cam plug in backwards. I theorized that soon after it's rebuild the camshaft carved what it had to have off the back of the plug and some of the "snarfles" found their way back through the cam bearing and into the main bearing. The engine as a whole apparently took the hit because it was a high mileage piece when I tore it down. Randy M

This is the reason I'm asking. This hole, is about 25% under the cam bearing.
If there were ever problems for #5's meandering route from the cam to the rear main, I was thinking of running a 3/16 ID line to it, as shown by the fittings. But if there are no issues for #5, I'll just angle the hole over, closer to the center of the recess.

Never had a problem there and I've never touched a die grinder to that area before. Solution looking for a problem that doesn't exist.

Same here, haven’t seen any issues. Feed looks strange but works

Agree with Brent and Ross, that annular groove is sufficent as is, I wouldn`t mess with it. Since it has been adequate for 50+ years, I think you have nothing to worry about.
