When a rod is forged, is the big end open to be machined to final size ??
Is the small end blank, drilled out or such and machined to final size ??
Cast or forged piston.... is the wrist pin hole blank and drilled out, machined to size ??

Rods are forged laying flat - most (not all) are forged with openings at both big and small ends that get subsequently machined to size.
Pistons are forged "sitting upside down" so pin holes need to be drilled and sized afterwards.

To add to what Barry said, Cast pistons are "usually" "near net" because that is how the molds are made. Forgings are often engine type specific , but still "generic" to allow flexibility of stroke and rod combinations by drilling the pin hole where it is need.
