I have a Holley that I believe is a 4150 ?
It does not have the brass tag.
The two numbers I read are below, but do not have the numbers before and after these numbers like shows on the link I provided.
I want info on exactly what carb this is, and to buy a rebuild kit. Some of the numbers on the site say it's for the Shelby GT 350.

Nope.... not for a GT350.
It's a 4160, 570cfm carburetor originally made for a 440 Dodge with an automatic transmission, built on November 21, 1969.

Ohh and when buying a kit, I think that carb has an accelerator pump transfer tube, so you'll need the orings for that.

Some carbs have a transfer tube between the metering block and main body for the accelerator pump.
These carbs have two orings for the tube and a different metering block gasket.

A 3259 is a Shelby carb but that's not your carb #.The list # is and yes its a 440 Dodge carb..

Will any 4160 570cfm carb rebuild kit work or do I need more numbers ?
Is it 1969 (+/-) era carb ?

But I'd make sure it has the accelerator pump transfer tube first.... just pop off the metering block

Is the link you provided have that provision or do I need some other part number ?

It has everything you need, assuming you have the transfer tube I mentioned (you need to take the metering block off to know, I simply don't recall, nor is it in my notes).
Only thing it does not contain are floats and VS diaphragm as I buy these elsewhere, tho in your case I think I'd look at the existing parts before replacement.... in my case the carbs are for sale, so it's just a good practice to change them.
